After the clinic has been selected and the test has been scheduled, ProScreening will email the electronic chain to the donor. The donor must take the electronic chain and their photo ID with them to the clinic.
If the donor has been scheduled at an out of network clinic and a physical chain of custody form is being shipped overnight, the donor will be instructed to wait for the physical chain of custody form before going to the clinic.
